Practice Plan Terms and Conditions

Annual:

The Practice Plan Annual Membership provides a 20% discount on eligible treatments. Membership fees are payable in full at the time of enrolment and are non-refundable.

Should the member fail to maintain payments or elect to cancel the membership prior to completion of the twenty four (24)-month term, any discounts previously applied may be rendered void and treatment fees may be reassessed at the prevailing standard rate.

Monthly:

The Practice Plan Monthly Membership allows patients to remit fees in twenty four (24) monthly installments. To ensure equitable use of membership privileges, benefits including the 20% discount shall become effective only upon completion of the initial three (3) months of continuous membership payments.
Should the member fail to maintain payments or elect to cancel the membership prior to completion of the twenty four (24)-month term, any discounts previously applied may be rendered void and treatment fees may be reassessed at the prevailing standard rate.

Abuse of Discounts

Members who utilise discounts exceeding the equivalent value of their membership fee and who do not renew for a subsequent term may be deemed to have misused the plan. In such circumstances, the Practice reserves the right to:
– Revoke future eligibility for any discounted membership plans; and/or
– Reassess treatment pricing retroactively at the standard rate.

The Practice further reserves the right to amend or terminate membership benefits or agreements at its sole discretion in cases of misuse, breach of terms, or cancellation outside of standard renewal procedures.
